Dear Parent or Guardian:
According to requirements from the Iowa Department of Health and Human Services, all students entering 7th grade need to have a Tdap and Meningococcal vaccine BEFORE the beginning of the school year.
Your child will not be able to attend school on or after Tuesday, September 26, 2023, if they have not received these vaccines.
Your child may receive these vaccines at their physician's office or at Henry County Public Health. The ONLY exceptions are a Medical Exemption or Religious Exemption (which will also need to be on file at school by Tuesday, September 27, 2022).

Every Tuesday 9:00-11:00 am
Every Wednesday and Thursday 1:00-4:00 pm
1st and 3rd Thursday of every month from 1:00-6:00 pm.
No appointment is necessary.
